# Tooth movement or a jaw change: what is the target?

General adult education with clearly labelled fictional explanations\. Your treating clinicians explain your individual findings and care\.

## Start with what the proposed change is meant to do

Orthodontic tooth work and a jaw\-position operation have different targets\. In combined care, the intended fit of the teeth connects those jobs\. A jaw\-surgery discussion therefore does not make the orthodontic explanation unnecessary\.

Leah and Noah below are two fictional adults with different clinician\-established targets\. Their explanations illustrate the meaning of the proposal, not a method for sorting yourself into a treatment group\.

## Leah: a proposed tooth\-position and contact change

Original fictional professional explanation

### What Leah is told

The clinician explains: In the assessment and proposal we have discussed, the work is directed at the positions of your teeth and their intended front overlap\. I am explaining an orthodontic contribution to that contact goal\. A change in jaw position is not included as a target of this proposal\.

The explanation continues: The appliance name is one part of how I would pursue the tooth\-position work\. Its role and limitations still need to fit your own findings and care responsibilities\. We will review the developing tooth contacts rather than treat an illustration of the goal as proof that it has happened\.

### Leah's completed interpretation

Leah can distinguish the means from the goal\. The goal is the stated front\-contact change; orthodontic tooth\-position work is the proposed contribution\. She has not been told that every adult open bite can be addressed that way or that a preferred appliance guarantees the goal\. Her next conversation can stay focused on the actual approach and review within her own proposal\.

## Noah: a jaw\-relationship target with connected tooth work

A separate original fictional professional explanation

### What Noah is told

Noah's fictional clinician explains: Your assessment identifies a jaw\-position problem relevant to the bite goal\. Orthodontics alone cannot correct that jaw\-position problem\. The combined proposal has a jaw\-relationship target as well as tooth\-position work, so the different contributions need to be explained together\.

The explanation continues: The orthodontic work concerns how your teeth are arranged in relation to the jaw change being considered\. The surgeon explains the proposed operation, its individual risks and recovery\. We need to explain the intended tooth fit and how our responsibilities connect, including any orthodontic work that remains within the combined plan\.

### Noah's completed interpretation

Noah now understands why tooth straightening alone would not describe the proposed target\. His explanation includes a jaw change and connected orthodontic work\. The two jobs share a bite goal but are not interchangeable\. The operation, sequence, alternatives and personal tradeoffs still belong to an individual discussion with his clinicians\.

## The difference is the assessed target, not which story looks like you

Leah's supplied proposal has a tooth\-position target\. Noah's supplied proposal includes a jaw\-position problem that orthodontics alone cannot correct\. Those established differences give the proposed work its meaning\. Neither a photograph nor the size of a visible gap supplies the same information\.

An appliance preference is worth discussing, but it cannot substitute for the target\. A clinician should explain what the proposed option can contribute and which part of the goal it does not address\. The existing [tooth\-and\-jaw target lesson](<https://orthodonticjawguide.com/tooth-position-and-jaw-relationship>) and [combined\-care responsibilities lesson](<https://orthodonticjawguide.com/different-jobs-in-combined-care>) develop those connections further\.

## Research describes treated groups, not your best method

The 2020 US PBRN adult study included aligners, fixed appliances, temporary anchorage devices and jaw\-surgery care\. Practitioners selected treatment; the groups were not randomized, and some were small\. Its contact measurements also captured different endpoints\.

Those observations show why a study group is not a personal treatment recommendation or an appliance league table\. A group's reported association cannot tell you that the same method is best for your findings\. Nor does a method's appearance in a study establish that a particular adjunct is offered or suitable at one clinic\.

## A complete explanation connects purpose and limits

Both fictional adults can now state the target and the intended contribution\. That is a useful result of the explanation itself, before any treatment result exists\. Read how Leah's clinician carries the goal into [progress review and later stability](</following-your-open-bite-plan>) \.
